During your initial appointment either Rita or Nadine will meet with the patient to discuss what brought you in, review your medical history form and take a panoramic x-ray and photos. Dr. Podhouser will then give the patient an exam and present probable treatment. He will also take the time to consult with your general dentist and other specialists involved to ensure that your care is optimally coordinated. Further diagnostic records and impressions may be recommended. In straightforward cases, the treatment plan is confirmed and fees presented at the initial visit. In a complicated case, Dr. Podhouser will take additional time to review the records and a consultation visit will be scheduled to present treatment options.
If you have dental insurance, bring your current insurance card. If you don’t have one, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim. IMPORTANT: All patients under the age of 18 years must be accompanied by a parent or guardian at the new patient visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
